Accident Summary:

 WO HENLEY AND HIS CREW RETURNED TO CITY FIELD IN BAN ME THOUT AFTER EXPERIENCING TWO SUDDEN JERKS TO THE LEFT IN THEIR AIRCRAFT AND TO CHECK THE MAIN GENERATOR. AT CITY FIELD, WO HENLEY SHUT THE AIRCRAFT DOWN WHILE SP5 PAYNE, A TECH INSPECTOR, INSPECTED THE AIRCRAFT. HE NOTED THAT THE IRREVERSIBLE VALVES WERE LEAKING EXCESSIVELY. HE TIGHTENED THE BOLTS UNTIL THE SEEPAGE STOPPED, AND DURING RUN-UP CHECKED THE AIRCRAFT FOR FURTHER SEEPING OF THE SERVOS. HE NOTED A SLIGHT GROWL FROM THE MAIN GENERATOR, AND INSTRUCTED THE CREW TO RETURN THE AIRCRAFT TO CAMP HOLLOWAY FOR FURTHER MAINTENANCE. ON CLIMBOUT THE AIRCRAFT YAWED TO THE LEFT AND A LOUD GROWLING SOUND WAS HEARD FROM THE TRANSMISSION AREA. THE PILOT TURNED RIGHT TO REMAIN IN CLOSED TRAFFIC TO RETURN TO CITY FIELD. AT THIS POINT THE AIRCRAFT YAWED AGAIN TO THE LEFT, THE ROTOR RPM STARTED TO DECREASE, THE PILOT NOTED THAT THE CYCLIC WAS LOCKED IN A FORWARD, RIGHT POSITION, AND THE COLLECTIVE WAS TRYING TO CLIMB. BOTH THE PILOTS CAME ON THE CONTROLS, BUT BOTH COULD NOT OVER RIDE THE CONTROLS. THE AIRCRAFT HIT THE GROUND GOING APPROXIMATELY 40 KNOTS, THE MAIN ROTOR BLADE STRUCK A TREE, THE TAIL ROTATED TO THE RIGHT AS THE AIRCRAFT SLID RIGHT FROM THE ACTUAL IMPACT POINT ANOTHER TWENTY FEET.\\
