Accident Summary:
THE AIRCRAFT COMMANDER WAS WO1 GARY V. BURNS AND THE PILOT WAS WO1 JACKIE R. LINDSEY. MR. BURNS STATED THAT AT 0940 HOURS ON 21 AUGUST 1969, HE RECEIVED A MISSION TO RESUPPLY A GROUND UNIT 1 1/2 MILES WEST OF LAI KHE AT GRID COORDINATES XT 6836. THE CREW WAS NOTIFIED AND A HASTY PRE-FLIGHT WAS ACCOMPLISHED. THEY ESTABLISHED RADIO CONTACT WITHT HE LOGISTICAL PERSONNEL WHO SUPPORTED "VODA 6" AT LAI KHE. THE LOGISTICS UNIT ASKED MR. BURNS IF HE WANTED INTERNAL OR AN INTERNAL LOAD. MR. BURNS DESIRED INTERNAL LOADING BECAUSE HE LACKED EXPERIENCE IN CARRYING SLING LOADS. THE AIRCRAFT WAS LANDED AT LAI KHE AT 1010 HOURS, AND WAS LOADED WITH DEMOLITION EQUIPMENT, 3 CRATES OF CONCUSSION GRENADES, AND 15 CRATES OF SHAPED CHARGES. A 25 FOOT HOVER CHECK WAS COMPLETED PRIOR TO TAKE-OFF WITH ALL INSTRUMENTS READING WITHIN OPERATING RANGE. AFTER DEPARTING LAI KHE, THEY ESTABLISHED RADIO COMMUNICATIONS WITH "VODKA 6", THE GROUND UNIT COMMANDER. "VODKA 6" TOLD MR. BURNS THAT HE WOULD HAVE TO HOVER OVER THE AREA AND NOT TERMINATE HIS APPROACH TO THE GROUND BECAUSE THE AREA WAS INADEQUATE. MR. BURNS TOLD THE GROUND COMMANDER THAT HE COULD NOT HOVER INTO THE AREA BECAUSE OF THE WEIGHT ONBOARD EXCEEDED THE POWER CAPABILITIES OF THE AIRCRAFT. THE GROUND COMMANDER ASKED IF MR. BURNS COULD SLOWLY FLY OVER THE CRATES OFF. MR. BURNS SAID HE COULD AND ASKED THE GROUND COMMANDER TO CLEAR THE AREA OF TROOPS. THE GROUND COMMANDER NOTIFIED THE CREW THAT THE AREA WAS CLEAR. MR. BURNS MADE ONE PASS OVER THE AREA AND THE CREWCHIEF THREW ONE 40 POUND CRATE OFF THAT LANDED 25 FEET SHORT OF THE INTENDED AREA. A SECOND PASS WAS INITIATED AND THE CREWCHIEF AGAIN THREW ANOTHER 40 POUND CRATE OFF. THE CREWCHIEF TOLD MR. BURNS HE THOUGHT THE CRATE HAD FALLEN ON A FRIENDLY TROOP. SHORTLY AFTER THIS, THE GROUND COMMANDER SAID TO STOP THROWING THE MATERIAL OFF BECAUSE A CRATE HAD HIT AN ARVN SOLDIER. THE GROUND COMMANDER ASKED MR. BURNS IF HE WOULD ATTEMPT TO HOVER INTO THE LZ. MR. BURNS MADE A HOVER CHECK 50 METERS TO THE RIGHT OF THE LZ AND AT A 30 FOOT HOVER OVER THE 100 FOOT TREES. ALL THE INSTRUMENTS WERE WITHIN OPERATING RANGE. MR. BURNS THEN TOLD THE GROUND COMMANDER THAT HE COULD HOVER INTO THE LZ. CONFIDENT OF THE SITUATION BECAUSE OF THE HOVER CHECK AND THE LOSS OF 80 POUNDS OF MATERIAL, MR. BURNS MANEUVERED THE AIRCRAFT TO THE LEFT AND DIRECTLY OVER THE LZ. HE PAUSED MOMENTARILY OVER THE LZ TO CLEAR THE TAIL ROTOR AND TIP PATH PLANE. THE AIRCRAFT THEN BEGAN A GRADUAL, VERTICAL DESCENT INTO THE LZ. THE AIRCRAFT BEGAN TO VIBRATE BADLY DURING THE DESCENT. AT APPROXIMATELY 40 FEET AGL, MR. BURNS TRIED TO SLOW THE RATE OF DESCENT AND SIMULTANEOUSLY HEARD THE ENGINE LOSE RPM. MR. BURNS GLANDED OVER THE ENGINE TACHOMETER AND IT READ 6200 RPM AND WAS DESCENDING THROUGH 6000 RPM (NORMAL OPERATING RANGE IS 6600 RPM). THE AIRCRAFT BEGAN TO YAW TO THE RIGHT WITH MR. BURNS APPLYING FULL LEFT PEDAL. MR. BURNS REDUCED POWER TO RETAIN WHAT ENGINE RPM HE HAD REMAINING. THE ENGINE TACHOMETER READ 5600 RPM. AS THE AIRCRAFT SETTLED INTO THE TREES, MR. BURNS USED THE REMAINING RPM TO CUSHION THE LANDING PRIOR TO GROUND CONTACT. THE ENGINE WAS SHUT OFF AND EVERYONE EXITED THE AIRCRAFT.\\
