Accident Summary:

 AIRCRAFT UH-1C SN 66-15140 HAD FLOWN ON A GUNSHIP MISSION AT BAM ME THUOT, RVN ON 30 OCTOBER 1968. ON THE RETURN FLIGHT, IN THE VICINITY OF PHU NHON, THE AIRCRAFT COMMANDER FELT A SHUDDERING SENSATION WHICH HE FELT EMANATED FROM THE TAIL SECTION. AT THIS TIME HE TOOK THE CONTROLS FROM THE PILOT AND CHECKED THE PEDALS FOR MOVEMENT AND CONSEQUENT AIRCRAFT RESPONSE. THERE WAS NO RESPONSE TO PEDAL MOVEMENT. BY THIS TIME THE AIRCRAFT HAD YAWED TO THE RIGHT EXCESSIVELY AND HAD APPROXIMATELY 15 DEGREES OF RIGHT BANK. THE AIRCRAFT COMMANDER WAS INFORMED BY HIS WING MAN AND CREW CHIEF THAT THE TAIL ROTOR WAS BARELY TURNING (WINDMILLING) AND THAT HE HAD, IN FACT, EXPERIENCED A TAIL ROTOR FAILURE. AT THIS POINT THE AIRCRAFT WAS AT APPROXIMATELY 3500 FT. MSL (1500 FT. ABSOLUTE), 80 KNOTS INDICATED AIRSPEED, AND IN A GRADUAL TURN TO THE RIGHT. THE AIRCRAFT COMMANDER ATTEMPTED TO FLY THE AIRCRAFT TO THE PHU NHON AIRSTRIP BUT WAS UNABLE TO RECOVER FROM THE RIGHT BANK ATTITUDE IN SPITE OF HIS INDUCING FULL LEFT CYCLIC PRESSURE. HE FURTHER ATTEMPTED TO STREAMLINE THE AIRCRAFT BY INCREASING THE AIRSPEED TO 90 KNOTS, BUT WAS UNABLE TO DO SO. AN INCREASE IN POWER ONLY TENDS TO INCREASE YAW TO THE RIGHT. A 40 TO 70 KNOT AIRSPEED IS THE MOST DESIREABLE UNDER THESE CONDITIONS. THE AIRCRAFT COMMANDER THEN DECIDED TO LOSE ALTITUDE AND ATTEMPTED AN AUTOROTATION INTO ONE OF THE OPEN FIELDS BELOW HIM. HE JETTISONED HIS ROCKET PODS AND INSTRUCTED THE CREWCHIEF AND GUNNER TO GET RID OF ALL EXCESS WEIGHT FROM THE CARGO COMPARTMENT. AT 70 KNOTS AND APPROXIMATELY 150 FEET THE AIRCRAFT BEGAN TO VIBRATE EXCESSIVELY AND TO YAW MORE TO THE RIGHT, GRADUALLY ENTERING COMPLETE 360 DEGREE TURNS. BEING OVER HIS SELECTED FORCE LANDING SITE, HE ENTERED AUTOROTATION. THE AIRCRAFT COMPLETED APPROXIMATELY THREE TURNS BEFORE IMPACTING.\\
