Accident Summary:
THE DAY OF THE ACCIDENT THE MAINTENANCE SHIP RETURNED TO THE DOWNED AIRCRAFT WITH THE PROPER PARTS, A TEST PILOT, TECHNICAL INSPECTOR, AND AIRCRAFT MECHANIC. THE AIRCRAFT WAS CHECKED FOR LEAKS BEFORE STARTING, AND AN OIL MANIFOLD, START FUEL LINE AND PRESSURE SWITCH WERE REPLACED. THE ENGINE TAILPIPE V-BAND WAS FOUND TO BE LOOSE AND WAS TIGHTENED, THIS CURED THE EXCESSIVE VIBRATIONS FELT ON 20 AUGUST BY THE AIRCRAFT COMMANDER. WO MILLS WAS THE PILOT OF THE AIRCRAFT AND WAS TO PERFORM ALL HOVER CHECKS AND FLY THE AIRCRAFT AS HE WAS A TEST PILOT FOR UH-1C AIRCRAFT IN HIS UNIT. WO MILLS STARTED THE AIRCRAFT ENGINE AND A HOVER CHECK WAS MADE, THEN THE ENGINE WAS CHECKED BEFORE SHUT DOWN ON TWO OCCASIONS PRIOR TO TAKEOFF. EACH TIME THE ENGINE AND AIR FRAME WERE CHECKED NO EXCESSIVE VIBRATIONS WERE FELT OR LOUD UNUSUAL NOISES HEARD. THE ENGINE WAS STARTED FOR THE THIRD TIME AND A HOVER CHECK WAS COMPLETED ONCE MORE THEN SSG FOX REPLACED ALL THE ENGINE COWLING AND CLIMBED INTO THE LEFT SEAT OF THE AIRCRAFT. THE AIRCRAFT WAS LOADED WITH 1000 PLUS POUNDS OF FUEL, APPROX 38 ROCKETS IN A XM=21 SYSTEM, TWO M-60 AMMO CANS FULLY LOADED AND ONE TOOL BOX WEIGHING APPROX 50 POUNDS AT TIME OF TAKEOFF. WO MILLS THEN ENTERED THE TRAFFIC OF PHOUC VINH AIRFIELD AND RADIOED TO WO NELSON IN THE CHASE AIRCRAFT THAT HE WAS GOING TO DEPART FOR CU CHI. AT 900 FEET AND 65-70 KNOTS THE AIRCRAFT EXPERIENCED AN ENGINE FAILURE. WO MILLS ENTERED AUTOROTATION IMMEDIATELY AND RADIOED HIS SITUATION TO THE CHASE AIRCRAFT. THE CHASE AIRCRAFT THEN FOLLOWED THE AIRCRAFT DOWN TO THE GROUND. THE AIRCRAFT COMMANDER THEN PICKED OUT WHAT HE THOUGHT TO BE THE MOST FAVORABLE AREA FOR A FORCED LANDING AND PROCEEDED TO MAKE A RUN ON LANDING. ON SHORT FINAL TO TOUCHDOWN WO MILLS NOTICED THAT THE AREA OF INTENDED LANDING LANDING WAS NOT SMOOTH, THAT FURROWS AND DIKES OR SMALL HILLS OF APPROX 3 FEET IN HEIGHT WERE HIDDEN BY THE GRASS. HE THEN ELECTED TO MAKE A ZERO GROUND RUN LANDING TO THE TOP OF ONE HILL AND THE FORWARD MOMENTUM OF THE AIRCRAFT CAUSED IT TO MOVE FORWARD AND SLIDE DOWN TO THE BOTTOM OF THE HILL, APPROX 3 FEET, WHERE IT CAME TO REST. THE CHIN BUBBLES WERE BROKEN AS A RESULT OF THIS IMPACT AND COUPLED WITH EXCESSIVELY LOW ROTOR RPM DUE TO PITCH PULL PRIOR TO LANDING, CAUSED THE MAIN ROTOR BLADE TO FLEX DOWN AND CUT OFF THE TAIL FIN JUST FORWARD OF THE 42 GEARBOX. THE PILOT THEN SHUT THE ENGINE DOWN AND WITH THE TI PROCEEDED TO UNLOAD ROCKETS AND EQUIPMENT ON TO AIRCRAFT 68-15575 AS THIS WAS AN INSECURE AREA.\\
