^ON 1 NOVEMBER 1968 THE UH-1H, SN: 66-16110^, BELONGING TO THE ^170TH ASLT HEL CO^, WAS BEING RETURNED TO ^HOLLOWAY ARMY AIRFIELD, RVN^. THERE HAD BEEN INCIDENT DAMAGE TO THE TAIL ROTOR THAT DAY DUE TO WIRE THAT SURROUNDS THE LOC, FLYING UP AND BECOMING TANGLED IN THE TAIL ROTOR. THE AIRCRAFT WAS INSPECTED IN ACCORDANCE WITH TM 55-1520-210-20, PAGE 3-8, A SUDDEN STOPPAGE-TAIL ROTOR, AND THE NECESSARY REPAIRS MADE AT THE LOC SITE. THE AIRCRAFT WAS INSPECTED BY ^SP/6 MARSOCCI^ AND FOUND TO BE ACCEPTABLE. ^LT. WILLIAMS, WITH SP/6 MARSOCCI^ PROCEDED TO START THE AIRCRAFT AND UPON HOVERING ^LT. WILLIAMS^ NOTED, AS HE DESCRIBES IT, A SLIGHT HIGH FREQUENCY VIBRATION IN THE ANTI-TORQUE PEDALS. THE AIRCRAFT WAS SET DOWN, SHUT DOWN AND ^SP/6 MARSOCCI^ AGAIN MADE A THOROUGH TECHNICAL INSPECTION OF THE ANTI-TORQUE SYSTEM. THE SHIP WAS ONCE AGAIN FOUND TO BE CORRECT BY THE TECHNICAL INSPECTOR. THE AIRCRAFT WAS STARTED, HOVER CHECKED, AND AT APPROXIMATELY ^1725^ HOURS THE AIRCRAFT STARTED TO TAKE OFF FROM THE ^OASIS^, ^(ZA105273)^. UPON REACHING 150 TO 200 FEET ABSOULTE AND 50 TO 55 KNOTS INDICATED FORWARD AIRSPEED A LOUD NOISE WAS HEARD, AND APPEARED TO COME FROM THE AREA OF THE TRANSMISSION. THE AIRCRAFT STARTED A SLOW RIGHT TURN. THE PILOT AND TECHNICAL INSPECTOR RECOGNIZED THE FAILURE OF THE ANTI-TORQUE SYSTEM. THE PILOT STATED ALL HIS EFFORTS TO CAUSE THE AIRCRAFT TO STREAMLINE WERE IN VAIN. MANIPULATION OF THE THROTTLE FAILED TO PRODUCE THE DESIRED RESULTS. THE AIRCRAFT IMPACTED THE GROUND AND ROLLED ON ITS LEFT SIDE. THERE WERE LITTLE OR NO SKID MARKS INDICATING LITTLE OR NO FORWARD MOMENTUM OR MOVEMENT. TAIL BOOM CRACKS AND THE COMPRESSION BENDS IN THE FIRST SECTION OF DRIVE SHAFTING INDICATE DOWNWARD MOVEMENT UPON IMPACT. THE LACK OF EVIDENCE OF SCORING ON THE DRIVE SHAFTING, ALSO THE COMPRESSION DEFORMATION OF THE FIRST SECTION OF DRIVE SHAFTING WHERE THE SHAFTING PASSES THROUGH THE BULKHEAD. ALL ABOVE INDICATES THE ANTI-TORQUE SYSTEM TO BE INOPERATIVE AT THE TIME OF IMPACT.\\
