Accident Summary:
AIRCRAFT DEPARTED THE CHRISTMAS TREE REVETMENTS AREA AND REFUELED AT THE HOLLAWAY HOT SPOT. THEN IT PROCEEDED TO AN AREA APPROXIMATELY TWELVE NAUTICAL MILES SOUTHWEST OF KONTUM AIRFIELD FOR A VISUAL RECON OF THE AREA. THE MISSION WAS TO LOCATE A PILOT MISSING FROM AN AIRCRAFT DOWNED ON 13 AUG 69. THE CREW THEN FLEW TO FOB II TO BORROW A MCGUIRE TO EXTRACT THE MISSING PILOT IF HE WAS LOCATED IN AN AREA WHERE THERE WAS NO SIT-DOWN LZ. THE AIRCRAFT THEN FLEW TO LZ MARY LOU WHERE IT WAS REFUELED, TOPPED-OFF BECAUSE OF NO FUEL GAUGE OR CLOCK, AND SHUT-DOWN WITH THE OTHER SEARCH AIRCRAFT. THIS INITIAL FLIGHT LASTED 55 MINUTES. APPROXIMATELY ONE HOUR LATER THE AIRCRAFT CRANKED TO GO TO LZ PENNY TO OBTAIN GROUDN TROOPS FOR SWEEPING THE AREA FOR THE DOWNED PILOT. BETWEEN FIVE AND TEN MINUTES AFTER TAKE-OFF THE AIRCRAFT COMMANDER FELT FEEDBACK IN THE CYCLIC AND CALLED THAT HE WAS LOSING HIS HYDRAULICS. A FEW SECONDS LATER HE CALLED THAT HE WAS TURNING BACK FOR KONTUM AIRFIELD BECAUSE ALL HYDRAULICS WERE GONE. AS THE AIRCRAFT TURNED 180 DEGRES WHILE ON CRUISE FLIGHT THE RIGHT FUEL BOOST PUMP WARNING LIGHT CAME ON, FOLLOWED IMMEDIATELY BY LOSS OF RPM FROM 6600 TO 4000, WITH THE N1 ALSO STABILIZED AT 50 PERCENT. THE PILOT HAD BEEN RECYCLING THE HYDRAULICS WHEN LOSS OF POWER OCCURRED. THE AIRCRAFT COMMANDER, WO1 HARPER, HAD NO RESTORATION OF POWER WHEN HYDRAULICS WERE RECYCLED AND NOW ENTERED AUTOROTATION FOR A FIELD THAT HE HAD CHOSEN TO BE SUITABLE FOR LANDING. AS THE AIRCRAFT COMMANDER DESCENDED THE AIRCRAFT FROM 1500 FEET AGL THE PILOT, PLACED THE GOVERNOR IN EMERGENCY POSITION AND THE PILOTS ATTEMPTED TO ROLL ON THROTTLE WITH NO INCREASE IN N1 OR N2. THE CREW AND OBSERVER SAT IN FRONT OF THE TRANSMISSION OF THE FLOOR AND WAS ASSISTED BY THE GUNNER IN FASTENING HIS SEAT BELT. NO SEATS WERE DOWN BECAUSE THE EXTRACTION RIG DOES NOT ALLOW ROOM FOR SEATS. BOTH PILOTS WERE ON THE CONTROLS DUE TO THE LOSS OF HYDRAULICS. AS THEY NEARED THE GROUND THE PILOTS FLARED AND WHEN A PITCH PULL WAS ATTEMPTED THE COLLECTIVE WOULD NOT COME UP. THE AIRCRAFT IMPACT WITH THE GROUND WITH 30 TO 40 KNOTS OF FORWARD AIRSPEED, THE TOE OF THE RIGHT SKID IMPACTING INITIALLY. THE AIRCRAFT BOUNCED, IMPACTED WITH THE GROUND AGAIN, RIGHT SKID FIRST, AND ROLLED OVER ON ITS RIGHT SIDE. THE TAIL BOOM WAS SEVERED FROM THE ARRCRAFT, PROBABLY DURING THE FIRST CONTACT WITH THE GROUND. THE OBSERVER EITHER SLIPPED UNDER OR WAS THROWN OVER HIS SEAT BELT AND WAS THROWN FROM THE AIRCRAFT CURING THE INITIAL IMPACT. THE AIRCRAFT'S RIGHT SKID STRUCK THE OBSERVER ON THE SECOND AND FINAL IMPACT. THE AIRCRAFT PINNED THE OBSERVER TO THE GROUND UNDER THE RIGHT SKID. THE GUNNER ATTEMPTED TO GRASP THE OBSERVER AS HE WAS THROWN FROM THE AIRCRAFT, BUT, AS HE LEANED TO HIS LEFT HIS OWN SEAT BELT BEGAN SLIPPING, CAUSING HIM ALMOST TO BE THROWN FROM HIS SHIP. THE HYDRAULICS HAD BEEN LEFT OFF AFTER TEH HYDRAULICS FAILURE AND THE PILOT TURNED OFF THE FUEL SWITCH. THE ENTIRE FOUR MAN CREW EXITED THE AIRCRAFT.\\
